1. MTI Claim form (Please fill in and upload all 3 pages)

2. Power of Attorney Mandate
Please use this naming protocol to name your document
(MTI NUMBER - yymmdd (date in reverse order) - YOUR FULL NAMES- POA)
3. Statement of Truth (Non-South African residents only)
Please use this naming protocol to name your document
(MTI NUMBER - yymmdd (date in reverse order) - YOUR FULL NAMES- SOT)
4. Copy of ID
Please use this naming protocol to name your document

(this can be your ID, Passport, anything proving your identity, not necessarily your South African Id)
5. Proof of bank account

(we require the bank letter/ statement to show the account name, sort code/ branch number and account number to effect payments into your account)
6. Proof of Transactions to attach.

7. Assignment-Form
